Preparing for your appointment:

1. Three forms are required to check your pet in for their appointment. These forms are emailed when you book an appointment. You may also print and fill them out at home and bring them to your appointment.

   Requested Services Form

2. Please withold food from your pet starting at midnight the night before surgery. Your pet may have water.

3. If your pet is a cat, please arrive between 7:00am and 7:15am on the day of your cat’s appointment.

4. If your pet is a dog, please arrive between 7:15am and 7:30am on the day of your dog’s appointment.

5. Please do not bring your pet into the lobby unless you have completed all three forms.

6. All cats need to be in a kennel. All dogs need to be on a leash.

7. Pick up is at 3:00pm. We may call you if your pet is fully recovered prior to 3:00pm.
